An Advance Care Directive, which is also known as a ‘living Will’, is a document which is often created at the same time as an Enduring Guardianship.

An Advance Care Directive records specifically the level of care and treatment you are willing to accept for yourself in the event that you may not be able to communicate those wishes to your family members or doctors. 

As with an Enduring Guardianship you must be 18 years of age and meet capacity requirements to create this document.
